GoAskAlex Endorses Emergency Contact Form for Performer Safety

LOS ANGELES — GoAskAlex is promoting performer safety by advocating for the implementation of an official Emergency Contact Form for independent models and content creators.
 
Dubbing it the "S'workerz Emergency Contacts," Alex said the form should include pertinent information that can be utilized in the event of an emergency, such as legal name, date of birth, medication requirements, allergies, blood type, pet placement information and personal requests for removal of any sensitive items from the home in the event of incapacitation.
 
“Recently, a colleague and I encountered an emergency and I realized that we didn't actually know each other's full legal names, birthdates — anything, really,” said Alex. “As a result of this, I’ve put together a detailed Emergency Contact Sheet for all of my local colleagues to fill out. This way, if the unthinkable were to happen and we needed to provide details to a medical professional or family member, we could do so more easily. I assume when studio performers are on set, the director has all of their personal contacts, but for those of us who work from home and network with local colleagues, this information is often overlooked and it's important to know these things.”
